The Compiled Military Service Records for Henry Simmons state he enlisted on March 31, 1865, at Bowling Green, Kentucky, and he mustered in on the same day. Henry was 30 years old, height of 5′ 8″, with brown eyes, black hair, and brown complexion. Henry served in the 12th Regiment of Heavy Artillery, USCT, Company M. During his service, in February 1866, he was “in confinement” at Paducah, Kentucky. Beneath this comment, it states: “Remark cancelled byline drawn through.” Henry mustered out on April 24, 1866, at Louisville, Kentucky.

This pension card provides supporting evidence for the name of Henry Simmons’ wife: Mariah Simmons. In addition, the date of the Invalid’s Pension application, Oct. 24, 1890, suggests Henry was still alive at this time. The date of the Widow’s Pension application, Feb. 28, 1903, suggests Henry died shortly before this date.
